Information from secondary sources supplemented any missing data to give hospitals as much credit as possible toward their Safety Grade.
By contrast, Virginia ranked among the top the five states this fall.
The Leapfrog Group, a national nonprofit health care ratings organization, released new Leapfrog Hospital Safety Grades. The hospitals have programs where physicians, nurses and team members communicate and collaborate to reduce safety risks.
Coastal Carolina Hospital scored a "C" from spring 2014 through spring 2016, but jumped up to an "A" on the last three reports. The number of hospitals that receive each letter grade is relatively consistent each year, although the latest report showed the smallest number of hospitals who received an F grade.
There were no reported F grades in the entire state.
Both Hilton Head Hospital and Coastal Carolina Hospital are owned by Tenet Healthcare. The score rates how well hospitals protect patients from errors, accidents, injuries and infections.

Beaufort Memorial Hospital went through the most progress of any local hospital, according to the Leapfrog report.
Howard University Hospital in Washington, D.C. Its hospitals didn't fare well in their first report, with just one out of 44-Howard County General Hospital in Columbia-earning an "A" grade, the fourth lowest percentage in the country behind North Dakota, Washington D.C., and DE and just ahead of NY.
Fifty-nine hospitals have earned an "A" ranking in each of the safety grades since its inception.
"Receiving the highest mark from the Leapfrog Group is a reaffirmation of Kaiser Permanente's commitment to high-quality health care and patient safety", Senior Vice President and Area Manager for Kaiser in the Central Valley area, said in a statement.
Memorial Hospital of South Bend, Lakeland Regional Medical Center in St. Joseph, and Parkview Regional Medical Center in Fort Wayne all received A grades. Its hospital safety grades, released every April and October since 2012, are aimed at encouraging patients to pay attention to safety when they choose hospitals.

Opponents say the proposal would hit minority groups particularly hard. "Shoring up our parks' aging infrastructure will do that". The higher cost may keep some visitors away, but it's not clear how the new fee increases will affect overall park attendance.
